- make a list of all of your medications to share with your doctor
- locate your photo ID and most recent insurance card and prepare to bring it with you to your visit
- be prepared to pay a copay at your appointment
- complete any required paperwork you were given
- contact us at least 24 hours in advance to reschedule if you cannot make your appointment
